The Health Award:
Recognizes an individual who has made a significant contribution to children in the fields of health, medicine or the sciences.
(adult & organization)

Nominee must have created, managed or otherwise supported a sustainable program which has significantly contributed to the IMPROVED HEALTH of children.

Nominee must do this work over and above their normal employment, OR work for little or no pay.

Nominee must have been doing this work for a minimum of 10 years.

Nominee must have an existing non-profit organization/foundation in good standing, which can receive grant funds if awarded.
The Youth segment of this award recognizes youth that are making extraordinary contributions to the lives of other youth.

Nominee must be under the age of 21 by the nomination submission deadline.

Nominee must be working with an existing non-profit organization in good standing, which can receive grant funds if Awarded.
The Leadership Award:
The Leadership Award is given to a youth who has chosen to take a leadership role in bringing positive change to their school or community. This youth will have demonstrated responsibility and drive, as well as initiative in starting new projects and programs.
The Community Service:
The Community Service Award is given to a youth who is truly selfless and dedicated to helping others. Though not necessarily in a leadership role, this youth has shown commitment to bettering their community by volunteering their time to a cause they are passionate about.
The Advocacy Award:
The Advocacy Award is given to a youth or organization who has committed time and energy lending their voice to a cause and bringing about change.
The Arts and Culture Award:
The Arts and Culture Award is given to a youth who demonstrates a passion for arts and culture, and shows their commitment to helping others understand the importance of arts in the community.
The Perseverance Award:
The Perseverance Award is given to a youth who has proven that personal initiative and determination can overcome any form of adversity. This youth will have demonstrated courage, and tenacity when dealing with the enormous obstacles that an average person would normally retreat from.
The Entrepreneurship Award:
Awarded to a young person who has successfully initiated a business or social entrepreneurship project through the development of an innovative product or service. Managed an entrepreneurial project for profit or for charitable causes. Displayed innovation and initiative in working towards an entrepreneurial goal, or exhibits inspirational leadership in project management
The Sportsmanship Award:
The Sportsmanship Award is given to a youth who is passionate about athletics and demonstrates sportsman-like qualities such as mentorship, coaching, planning and enthusiasm. This Award is given to a youth who has demonstrated selflessness in regards to bringing about positive change for their team. This youth will have demonstrated compassion and kindness, as well as the qualities anyone could ask for in a best friend at the moment you need them the most.
The Scholar-Athlete Award:
Recognizes excellence in the classroom by Central Ohio student-athletes who are juniors or above in academic standing with a 3.5 or higher cumulative GPA. Nominations received should be from coach or teacher at respective school or organization. Nominee must have earned at least two varsity letters, through athletic participation in sanctioned sports during the complete school. If registered in a club; should have participated in 2 all-team tournaments/championships.
ENTRY REQUIREMENTS

Nominees must reside within the Franklin County/Columbus-city limits

Anyone, youth and adults alike, can nominate a young person for an award!
NOMINEES SHOULD BE ABLE TO:

Complete our nominee questionnaire and return it with a photo of themselves by the deadline to complete the nomination process

Attend the awards ceremony if short-listed
In addition, short-listed nominees will be expected to:

Attend the awards ceremony November 20th, 2015

Have their photo taken which will used on the website and any publicity material that we may issue

Take part in any interviews prior to, during or after the event
All short-listed nominees receive a complimentary ticket for themselves & 1 guest to the event. Additional guests may purchase tickets through the LeaderSpark website.
Nomination Process
Youth can be nominated by either an adult or self nomination. Nominees are required to agree to the nomination. The nominator is responsible for submitting the nomination packet and must be available for inquires by the judging panel.
Applications must be completed and postmarked by October 1, 2015. Nominator must provide a short letter of recommendation detailing why the nominee should receive award. Complete the application and attach the required additional information. The judging panel reserves the right to interview any nominee prior to award selection. An individual or group cannot receive an award in the same category if they received the award in the previous year.
